ABSTRACT

SUMMARY OBJECTIVE: The aim of this study was to assess the inter-reliability of the Braden scale and its subscales for different patients assisted in the intensive care unit. We hypothesized that the Braden scale has low reliability in different populations. METHODS: This reliability study involved the Braden scale in intensive care unit of a hospital. A total of 200 patients were admitted to the intensive care unit in four different groups: neurological patients, sepsis, elderly, and adults affected by trauma. The Braden scale is a tool composed of six subscales for patient assessment: sensory perception, humidity, activity, mobility, nutrition, and friction. The total score was also calculated. The Braden scale was applied by two different nurses with an interval of 20-30 min between applications. RESULTS: For all populations, kappa values considered unsuitable were observed for most categories of the Braden scale, ranging from 0.06-0.25. Only for the total Braden scale score was moderate reliability identified in all groups evaluated, with intraclass correlation coefficient values ranging from 0.48-0.75. CONCLUSIONS: Braden scale is not a reliable tool to be used in the intensive care unit, and we do not recommend the use of this scale to assess the risk of developing pressure injury.

RESUMO Objetivo: construir e validar checklist para organização de salas operatórias. Método: estudo metodológico, desenvolvido entre junho e dezembro de 2018, em um hospital privado no estado do Maranhão - Brasil. Composto pelas seguintes etapas: revisão da literatura para construção do checklist, elaboração do instrumento e validação do conteúdo. O conteúdo foi validado pela avaliação de 74 componentes da equipe cirúrgica. Utilizou-se teste de confiabilidade pelo coeficiente Alfa de Cronbach e grau de consistência interna ≥ 0,70. Resultados: elaborou-se um instrumento com três itens (funcionalidade dos equipamentos/acessórios fixos e móveis da sala operatória; cirurgia; anestesia) e 33 subitens. O alfa de Cronbach total foi de 0,956. Conclusão: o checklist apresenta alta confiabilidade, boa consistência interna e adequação à organização na montagem das salas operatórias. Contribui na gestão de incidentes, na assistência cirúrgica segura e em subsídios para novos estudos.

Se compuso de los siguientes pasos: revisión de la literatura para la construcción de la lista de verificación, elaboración de un instrumento y validación del contenido. El contenido fue validado por la evaluación de 74 componentes del equipo quirúrgico. Se utilizó la prueba de fiabilidad mediante el coeficiente Alfa de Cronbach y el grado de consistencia interna ≥ 0,70. Resultados: se elaboró un instrumento con tres ítems (funcionalidad de los equipamientos/accesorios fijos y móviles en el quirófano; cirugía; anestesia) y 33 sub-ítems. El alfa de Cronbach total fue de 0,956. Conclusión: la lista de verificación presenta una alta fiabilidad, buena consistencia interna y adecuación a la organización en el montaje de los quirófanos. Contribuye a la gestión de incidentes, a la seguridad de la atención quirúrgica y a las subvenciones para nuevos estudios.


ABSTRACT Objective: To construct and validate a checklist for the organization of operating rooms. Method: Methodological study conducted from June to December 2018 in a private hospital in the state of Maranhão, Brazil. It consisted of the following steps: literature review for the construction of the checklist, elaboration of the instrument and content validation. Seventy-four (74) members of the surgical team made assessments to validate the content. Reliability test by Cronbach's alpha coefficient and degree of internal consistency ≥ 0.70 were used. Results: An instrument with three items (functionality of fixed and mobile equipment/accessories in the operating room; surgery; anesthesia) and 33 sub-items was developed. Cronbach's alpha coefficient was 0.956. Conclusion: The checklist has high reliability, good internal consistency and is suitable to the organization of the preparation of operating rooms. It contributes to a better management of incidents, safe surgical care and support for new studies.

ABSTRACT

O objetivo deste trabalho éavaliar a qualidade da conservação das vacinas em Unidades da Atenção Primária à Saúde em uma capital do Nordeste brasileiro. Trata-se de um estudo descritivo e avaliativo, transversal, com abordagem quantitativa. Aplicou-se parte do instrumento PAISSV 2.0 do Programa Nacional de Imunização, com profissionais da enfermagem entre agosto de 2016 e janeiro de 2017. Utilizou-se análise estatística descritiva. Avaliou-se 48 (92,3%) salas de vacinas. Na dimensão estrutura, destaca-se a ausência de câmaras refrigeradas e de instrumentos de monitoramento e controle da temperatura em 100% das salas avaliadas. Quanto ao processo de conservação, evidenciou-se que nenhuma das salas realizava a distribuição das vacinas nas prateleiras, conforme recomendações técnicas. 43 (89,6%) salas foram classificadas como não adequadas e nenhuma considerada adequada. A prática da conservação de vacinas ainda é incipiente, com inadequação da estrutura e processo de trabalho. Estas lacunas podem comprometer a erradicação e o controle das doenças imunopreveníveis.


The preservation of vaccines in Primary Health Care units in São Luís, a capital city in northeastern Brazil, is evaluated. Current descriptive, evaluative, cross-sectional quantitative approach was applied to a section of PAISSV 2.0 of the National Immunization Program with nursing professionals, between August 2016 and January 2017. Descriptive statistical analysis was employed. Forty-eight (92.3%) vaccine rooms were assessed. The complete absence of refrigerated chambers and the lack of monitoring and control instruments for temperature may be highlighted. In the case of the conservation process, not a single vaccine room distributed vaccines on shelves as recommended. Further, 43 vaccine rooms (89.6%) were classified as unsuitable and all were inappropriate. The vaccine conservation practice is still incipient, with inadequate structure and working process. These flaws may compromise the eradication and control of immunopreventive diseases.

ABSTRACT

Abstract INTRODUCTION: Chikungunya (CHIK) is caused by the Chikungunya virus, which is an Alphavirus of the Family Togaviridae transmitted to humans through female mosquitoes of the genus Aedes. METHODS: A cross-sectional study was conducted involving the administration of a questionnaire addressing sociodemographic and health variables and the Roland-Morris Disability Questionnaire on general pain to patients with CHIK in the City of Imperatriz, Brazil, between January and December 2017. RESULTS Data of a total of 130 patients were evaluated. The mean age was 52 years (standard deviation=13.3); majority of the patients were female (n=120) with a prevalence of 38.0% for functional disability. Statistical differences were noted for marital status (p=0.037), presence/absence of comorbidities (p=0.050), and the use of medications prior to the diagnosis of CHIK (p=0.050), use of methotrexate (p=0.030), use of nonsteroidal anti-inflammatory drugs (p≤0.035), and use of nonhormonal anti-inflammatory drugs (p=0.001). CONCLUSIONS Patients in the chronic phase of CHIK present functional disability, thus alerting healthcare professionals to the importance of implementing actions aimed at an adequate treatment in all phases of the disease, mainly related to pain treatment and motor rehabilitation.

Resumo OBJETIVO Avaliar o conhecimento e as práticas de biossegurança adotadas por profissionais do segmento da beleza. MÉTODOS Pesquisa descritiva do tipo survey. Foram entrevistados 238 profissionais de serviços de embelezamento entre agosto de 2014 e 2015. As variáveis foram apresentadas por meio de frequências absolutas e relativas, bem como média e desvio padrão. RESULTADOS 62,6% dos profissionais tiveram contato com sangue de clientes sem uso de luvas; instrumentais para o atendimento e 32,8% dos entrevistados não utilizaram equipamentos de proteção individual durante suas atividades laborais. CONCLUSÕES As doenças mais citadas quanto ao risco de contágio e de transmissão na prática laboral foram as hepatites virais, HIV e fungos. Quanto aos procedimentos de biossegurança adotados, o autoclave é o equipamento menos usado na esterilização dos instrumentos.


Resumen OBJETIVO Evaluar el conocimiento y las prácticas de bioseguridad adoptadas por profesionales del segmento del embellecimiento. MÉTODOS Evaluar el conocimiento y las prácticas de bioseguridad adoptadas por profesionales del segmento del embellecimiento. RESULTADOS: 62,6% de los profesionales tuvieron contacto con sangre de clientes sin el uso de guantes; el 74,4% higienizaban las manos entre los atendimientos, el 16,8% reutilizaban materiales desechables. Ningún profesional informó la cantidad adecuada de instrumentos y el 32,8% de los entrevistados no utilizaban equipos de protección individual durante sus actividades laborales. CONCLUSIONES Las enfermedades más citadas con respecto al riesgo de contagio y de transmisión en la práctica laboral fueron las hepatitis virales, el VIH y los hongos. Acerca de los procedimientos de bioseguridad adoptados, el autoclave fue el aparato menos utilizado en la esterilización de los instrumentos.


Abstract OBJECTIVE To evaluate the knowledge and biosafety practices adopted by professionals of the beauty segment. METHODS Descriptive study, of survey type. 238 professionals of the beauty segment were interviewed between August 2014 and 2015. The variables were expressed by means of absolute and relative frequencies, as well as average and standard deviation. RESULTS 62.6% of the interviwed professionals reported having had contact with blood from customers when they were not wearing gloves; 74.4% said they washed their hands before and after each service, and only 16.8% of the respondents reported reusing nonsterilizable materials. None of them was able to inform the correct number of sets of tools needed , and 32.8% of the respondents did not use Personal Protective Equipment during their work activities. CONCLUSIONS The most frequently reported diseases associated with the risk of infection and transmission in the work activities were viral hepatitis, HIV and fungi. Regarding the biosafety procedures adopted, autoclave is the least used method for sterilizing devices. Keywords: Beauty and aesthetics centers. Exposure to biological agents. ABSTRACT

Leishmania infection in humans, dogs and sandflies was examined in the endemic visceral leishmaniasis (VL) municipality of Raposa, state of Maranhão, Brazil. In this study, we examined Leishmania chagasi infection in the blood serum of both humans and Canis familiaris and the natural Leishmania sp. infection rate in the sandfly vector, Lutzomyia longipalpis. Enzyme-linked immunosorbent assay, indirect immunofluorescence reaction and polymerase chain reaction were performed to detect Leishmania infections in humans, dogs and sandflies, respectively. Overall, 186 out of 986 studied human beings were infected with L. chagasi parasites, representing an infection prevalence of 18.9 percent. An even higher infection rate was detected in dogs, where 66 (47.8 percent) out of 138 were infected. Among all Lu. longipalpis captured (n = 1,881), only 26.7 percent were females. The Leishmania infection frequency for the vector Lu. longipalpis was 1.56 percent. Remarkably, all infected sandflies were found in the peridomiciliary area. Furthermore, a high incidence of asymptomatic forms of VL in the human and canine populations was observed. The results of this study suggest autochthonous transmission of L. chagasi in this endemic area for visceral leishmaniasis because infection by Leishmania sp. was identified in all important elements of the transmission chain.
